During your first visit, bring all pertinent medical records, test results and a list of current medications. For your convenience, please download and complete the DISC New Patient Form prior to arrival. We also ask that you bring your insurance card, photo identification and co-pay. The office accepts Visa, Discover, MasterCard and cash.

For the benefit of all providers and patients, we ask that cancellations be called into the office at least 24 hours in advance.
